Waiving the school-sponsored Insurance
If you are already covered by comparable private health insurance you can waive the school sponsored plan. Please log on to the website below and complete the waiver process. You will need information from your current health insurance card to complete the waiver process. Waivers received from 2/16/12 through 2/29/12 will incur a financial penalty of no less than $131.00 and may not be accepted. After the end of the open enrollment period, only those cancellations as a result of qualifying life events can be considered and are dependent upon approval by the carrier.
Enrolling in the school-sponsored plan
To expedite the issuance of your health insurance ID card, please visit the website listed below and complete the enrollment process. The purchase of the student policy provides a one year term of insurance with effective dates 1/1/12-8/1/12 at a cost of $535.
Go to www.hulseqm.com/myinsurance and access your school’s information by using the
following:
Login ID: The student’s last name first initial entered without any spaces
Example: Enter John Smith as smithj
Password: The last 5 digits of your La Roche Student ID

insurance waivers and enrollments.
If you submitted your enrollment or waiver request more than one week ago and have not
received a confirmation, either at the end of the web process or via email, please resubmit
your information.
La Roche College is committed to meeting the needs of our students. In the past, we have had a
mandatory student health insurance policy for all athletes, international students and dance majors
and minors that provided access to quality student health insurance. Now the Affordable Care
Act of 2010, known as health care reform, further requires specific insurance benefits at certain
levels.
As a result, La Roche is taking proactive steps to ensure the school-sponsored plan is compliant
with these requirements. We are also expanding our mandatory student health insurance policy to
include all full time students taking 12 or more credits. By partnering with the health insurance
carrier and Hulse/QM, we ensure La Roche students have access to an ACA-compliant policy
that costs less than what can be found in the marketplace for comparable, individual health plans.
We recognize the increase in the cost of insurance will affect everyone over the next few years
and we are providing this service to assist students in controlling their health insurance
expenditures.
