Health Insurance
All students taking 12 or more credits must enroll in or waive out of the health insurance coverage offered through the College. All students, providing verifiable proof of comparable health insurance coverage, may waive the purchase of the sponsored plan.
At this time your student account has been billed $812.00 for the health insurance, including administrative fees. If you do not provide the necessary proof you will be automatically enrolled in the plan offered by the College. Any waivers requested after 2/15/13 will be assessed a financial penalty of no less than $133.00. Requests to waive after 2/28/13 cannot be accepted.
1. If you wish to enroll in the Insurance, please log on to hulseqm.com/myinsurance
and complete the enrollment process. This will expedite the receipt of your medical ID card. The purchase of the student policy provides health insurance with effective dates 1/1/13-8/1/13. After the end of the open enrollment period (2/28/13) only those cancellations as a result of qualifying life events can be considered and are dependent upon approval by the carrier.
2. To receive a credit for the $812.00 charge on your student account, please log on and complete the Waiver process. You will need information from your current health insurance card to complete the waiver process.
Please provide a current United States Address during this process.
Failure to respond by 2/15/13 will incur a financial penalty of no less than $133.00 that will not be removed. Requests to waive after 2/28/13 cannot be accepted. Go to www.hulseqm.com/myinsurance and access your school’s information by using the following:
Login ID: The student’s last name first initial entered without any spaces
Example: Enter John Smith as smithj
Password: The last 5 digits of your La Roche Student ID number
